The City of Humboldt encourages citizens to report non-emergency issues using the form below. Whether the issue is a pothole, damaged street signs, or a request for information, the City wants you to be part of the solution by reporting issues in the community.  Simply complete the form below with as much details possible. You can even send us a photo!  It’s that easy!

After Hours: Submissions received from here are monitored during regular business hours, Monday to Friday. If you are reporting a public works problem, such as water or sewer issues, and our office is closed, please call 306.682.2525 and press 1.
